Becoming a New Client

We’re glad you’re here! Taking the first step toward counseling is a brave and important decision.

At Samaritan, we’re committed to making the process as smooth and supportive as possible. On this page, you’ll find everything you need to know about what to expect, the next steps to get started, and how we’ll work together to match you with a therapist who can support your journey toward healing and growth.

How to Get Started

Call us at 608-663-0763 ext. 100 and leave a voicemail, or send us a message using the secure contact form below.

Our Intake Coordinator will contact you to collect basic details to help match you with the right therapist.

If we find a good match, we’ll send you new client paperwork through our secure system, TherapyNotes. If not, we’ll suggest other options.

Once your paperwork is complete, contact us to schedule your intake session.

Getting to Know You

Completing this form is optional. If you prefer, you can call us directly at 608-663-0763 ext. 100 to reach our Intake Coordinator for new client inquiries. If you choose to use the form, please provide the requested information to help us match you with a therapist who has current availability.

Questions About Our New Client Process?

We’ve got answers below! If you have a question not listed here, please feel free to reach out and we’d be happy to help.

1.

Do you take my insurance?

Our clinic works with most major insurance companies. If you have questions about coverage, we recommend calling your insurance directly for benefit and eligibility information.

2.

How do I complete the intake paperwork?

We use a program called TherapyNotes. You’ll receive an email from noreply@therapyportal.com with a link prompting you to create a client portal. The account will be linked to your email, and once set up, all the paperwork can be filled out electronically.

3.

In person or telehealth?

Most of our therapists offer both. Most insurance providers require an in-person visit at least once per year. You and your therapist will work out what suits you best.

4.

What should I expect during the intake session?

Your first visit is about 55-60 minutes. As a State Certified clinic, therapists are required to gather background information and a detailed history during the intake. Future appointments will focus more on the therapy process itself.
